U.S. VITAL RECORDS INFORMATION
- When applying for Italian Citizenship, all U.S. birth, marriage, death, certificates must be in “CERTIFIED COPY” a.k.a. “LONG FORM” or “FULL FORM” OR “BOOK COPY” (not “abstract”). These certificates are obtained from the Office of Vital Statistics of the State or County in which the event took place. Church Records are not accepted under most circumstances. Official Birth Certificates reporting only the “County” of birth will not be accepted. Birth Certificates must include the city or town of birth and include the names of both parents.
- If the information on the marriage license and certificate does not include full names of both spouses and dates of birth, the Consulate may require you to obtain the "Application for Marriage License."
- It is advisable to let the Office of Vital Records know that you will be having the certificate authenticated with the Apostille as some offices will include an Exemplification or Notary Statement with the Certificate. In some States this is needed in order to authenticate the document with the Apostille.
